Job Description

PRIMARY OBJECTIVE:  

Responsible for performing professional clinical review, processing, dispensing and compounding of prescribed specialty medications and supplies needed for patient care.  

 

MAJOR RESPONSIBILITIES:

·         Supports cross functional teams by modeling, coaching, and maintaining high standards in patient service. 

·         Contributes operational expertise in the execution of specialty care therapy. 

·         Collaborates with internal and external teams to develop differentiators and unique positioning of Specialty pharmacy management that enhance patient outcomes. 

·         Assists in maintaining an organized workflow system for providing efficient patient care.   

·         Reviews and processes prescription orders from authorized prescribers. 

·         Answers questions and provides information to patients and employees on drug interactions, side effects, dosage, administration and storage of pharmaceuticals and supplies.

·         Reviews, oversees and validates the work completed by pharmacy technicians engaged in all areas of the prescription fulfillment process including processing and claim submission of pharmaceuticals and/or supplies.

·         Coordinates with other pharmacies and pharmacists to assure all prescriptions are filled in a timely and professional manner and all required professional services are provided to patients and prescribers.

·         Completes patient counseling in compliance with clinical program protocols and documents any and all interactions with patients’ and prescribers in the appropriate systems in compliance with protocols.

·         Reviews patient demographics and prescriptions issued by physicians, or other authorized prescribers to assure appropriate therapy for the patient, accuracy of dose and medications and/or administration supplies needed for optimal patient outcomes.

·         Completes patient records, including initial patient assessment, medication profiles, care plans, initial opening patient notes and continuing patient progress notes.

·         Follows established procedures concerning security of patient information and general security and professional conduct of pharmacy operations as applicable.

·         Participates in the coordination of care to meet patient outcomes including, initial referrals, medication review and selection, assessment of medication delivery system, coordinated care conferences, patient care plans, compliance programs and discharge of patients from services.

·         Serves as a drug information resource. Actively researches and stays up-to-date on all disease states; also teaches these to all staff members.

·         Executes clinical pharmacy services including completing a thorough drug class review, evaluating current methods of handling specialty medications, recommending guidelines for appropriate clinical use as well as methods for reducing costs, reviewing prescriber requests for high-cost clinically complex agents, and collaborating with multiple vendors to obtain the best price possible. 

·         Supports the training of personnel.

·         Follows and ensures compliance with all company policies and procedures.

·         Monitors key performance indicators to measure, report and improve on time delivery performance to patients, group homes and hospitals and customer satisfaction. 

·         Supports and follows policies and procedures designed to identify and minimize profit loss.

·         Provides pharmacist support to the business in responding to company needs, company presentations, brainstorming program ideas in the specialty pharmacy management process or clinical process of Comprehensive Medication Reviews. 

·         All other duties as assigned.

 

DESIRED MINIMUM QUALIFICATIONS

 

Knowledge, Skills and Abilities:

  • Doctor of Pharmacy degree
  • Currently licensed or willing to consider licensure as a Registered Pharmacist in multiple states, including all states organization operates in. Current and unrestricted clinical licensure is a requirement. 
  • A minimum of two years as a licensed Pharmacist and a preferred minimum of two years of professional experience as a Specialty Pharmacist; or, a combination of Residency training, education, and experience that is equivalent to the employment standard listed above and that provides the required knowledge, abilities, and license. 
  • Must understand Specialty Pharmacy drugs including REMS and Limited Distribution Product best practices.
  • Clinical background in Specialty Pharmacy focused disease states preferred. 
  • Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the ambulatory care patient in collaboration with physicians, nurses and other healthcare professionals.
  • The ability to manage and/or contribute to multiple projects, both individual and team-based. 
  • Ability to make sound decisions. 
  • Excellent written and verbal communication skills, a collaborative work style and an entrepreneurial spirit.
  • Ability to use Microsoft Office products and execute basic PC skills.  
  • Ability to read and converse fluently in English.
  • Ability to maintain accurate records.
  • Ability to effectively meet and deal with the public in sales situations.
  • Ability to perform basic and business math calculations accurately.
  • Ability to handle stressful situations and deal successfully with difficult customers.
  • Ability to maintain effective working relationships with employees and supervisors.
  • Ability to understand and carry out written and oral instructions and ability to focus on details.
  • Availability to work evenings, weekends, and holidays when needed.
  • Good attendance and punctuality is required in order to fulfill the essential job functions.
  • Maintain confidentiality of patient information at all times.
  • Understands laws concerning Continuing Education requirements and takes responsibility to accomplish them.
