Position : Whole Person Care Provider

Organization : MyCHN – A nonprofit healthcare leader with 100+ providers and 15+ locations in the Greater Houston and Gulf Coast area

What You’ll Do:

Diagnose, treat, and manage patient care with autonomy
Collaborate with physicians & care teams
Educate and empower patients
Drive quality outcomes and innovation
Keep documentation sharp and timely
Float Providers: Travel between clinics to fill in where needed
Follows PCMH guidelines and principles, integration of services, and whole person care

What You’ll Need:

Licensed as Family Nurse Practitioner with recognition as Advanced Practice Nurse or licensed as a Physician Assistant with recognition by the Texas Medical Board
Master of Science in Nursing
Certification in area of practice or eligibility for certification
Must possess or be eligible to obtain a DEA license.
CPR/BLS Certified. ACLS a plus!
Bilingual in Spanish? That’s a big plus!
Advanced clinical skills & prescriptive authority
Compassion for all communities
Multitasking mastery & team spirit
